Dr. Shukla's entry into the field of medicine started with his MBBS from the MLN Medical College, Allahabad. He, then, went on to do his MS from the BRD Medical College, Gorakhpur, which was followed by Fellowships in Bariatric & Metabolic Surgery and Laparoscopic & Endoscopic Hernia Surgery.

According to the Arthritis Foundation, for  people with a lot of weight to lose, bariatric surgery can yield dramatic improvement.
In obesity due to Excess weight  causes -

  • Much pressure over joints and creates substances that can destroy cartilage, worsening osteoarthritis. The hips, knees, and lower back are usually affected the most. For every pound extra weight it increases 5-pound pressure across the joint when walking.

 Impacts of surgery -

  • Pain reduction -After bariatric surgery, pain reduces significantly and dramatically as weight loss occurs and it is not a surprise for patients that pain medicine is not required.

  • Increased mobility - Losing weight helps people to be more active and be able to do exercise and hence maintain the weight loss for long term.

  • Better outcome after joint replacement  - Patients (obesity with arthritis) who needs joint replacement surgery should  be reduce weight first by bariatric surgery because it helps to reduce post-operative problems and fasten recovery  after replacement surgery.

What is Diabesity?

Obesity and type-2 diabetes are global health problems that have a strong co-relation between them. The relative risk of diabetes with increased BMI is greater than those with normal BMI range. Generally 50% individuals are obese who are diagnosed with type 2 diabetes. Prevalence of diabetes increases disproportionately as obesity increases in  a population and now India is known as “Diabetes capital” of  the world and introduction of term ‘‘diabesity” was coined to understand that its a single disease.

What is Metabolic syndrome?

Defined as a combination of Increased Fasting Blood Sugar, Blood Pressure, Cholesterol and Triglycerides with abdominal Obesity that result in increased risk of Type2 Diabetes mellitus and Cardiovascular diseases.

What is diabetes surgery or metabolic surgery and it's type?

Surgeries which improves or cure diabetes and maintain blood sugar level within acceptable normal range and it can also correct lipid level in blood.

There are two types of surgery

  • One is pure for treatment of Type-2 Diabetes without obesity like Ileal Interposition, Ileal Interposition with Sleeve Gastrectomy and Ileal Interposition with Sleeve Gastrectomy and Duodenal switch
  • Second is Bariatric surgeries Like Sleeve Gastrectomy, Gastric bypass, Minigastric bypass, BPD with DS and Gastric banding also known as diabetic surgery or metabolic surgery because surgery improves or cures diabetes, hypertension and lipid abnormalities.

Mechanism of diabetes surgery
* By Change in gut hormone level and weight- loss insulin sensitivity and production increases thus control blood sugar level.                 

Criteria for  Baritric Surgery in type 2 diabetes (with Obesity)
*BMI >32.5 kg/mt2
*BMI >27.5 kg/mt2 and < 32.5 kg/mt2 with insulin requirements >80 IU /day
*Diabetes diagnosed < 5 years but results are also goods even <10 years old Type-2 Diabetes
*Failure to lose weight by diet and exercise

Impacts of Bariatric Surgery over Type-2 Diabetes
Different bariatric procedures can improve or cure diabetes and previously diabetes is considered as a progressive non-curable disease. But now curable by bariatric surgery or metabolic surgery and different procedure gives variable results as remission of Type2 diabetes mellitus by BPD with DS is 95%, by RNYgastric bypass is 85-90%, by Sleeve gastrectomy is 80% and by Gastric banding is 60%. In some cases our patients  discharge with normal blood sugar level and without antidiabetic tablets or insulin injection.
Overall cure of type2 diabetes is 85%, improvement  of type2 diabetes is 90 % and health and life quality improvements after surgery and requirement of drugs reduces or no need of drug are seen.

Criteria for diabetes Surgery (without Obesity)


  • Type 2 Diabetes more than 1 year duration
  • BMI 21-55 kg/mt2
  • Poor Glycemic control(HbA1c >8%) despite maximal use of Oral Hypoglycemic Agents and Insulin or good control but requiring progressively higher drug doses.
  • Gives Informed consent for the surgery
  • Postmeal C-peptide >1.0ng/ml
  • Age 25-75 years
  • Stable weight for more than 3 months

Impact of Diabetes Surgery over Type -2 diabetes

  • Approximately 90% cure or improvements are seen and there is no need for oral hypoglycemic and Insulin.

Bariatric surgery indicates a variety of gastrointestinal (GI) surgical procedures originally designed to induce weight reduction in morbidly obese patients. Benefits of bariatric surgery, however, extend well beyond weight loss and include dramatic improvement of type 2 diabetes, hypertension, dyslipidemia, and reduction of overall mortality.

Criteria for Bariatric Surgery (Weight loss Surgery)

  • BMI > 37.5kg/mt2
  • BMI 32.5kg/mt2 or >  with associated  problems like diabetes, hypertension, lipid abnormalities, artritis, Infertility, Breathing difficulties etc.
  • Age between 18-65 years

How Does Bariatric Surgery Work?

Today there are several bariatric surgical procedures that lead to sustained weight loss in patients with morbid obesity. These are classified by their presumed mode of action as below

  • Restritive procedure – restrict on the intake quantity of food i.e. less amount of calories intake e.g.- Intra-gastric Ballooning/Gastric Banding/Sleeve Gastrectomy
  • Malabsorptive procedure – to decrease absorption of digested food i.e less calories absorption e.g.-Bilio-Pancreatic Diversion/Duodenal switch
  • Combined procedure – i.e.-both restriction and malabsorption of calories e.g-Roux-en-Y-Gastric-Bypass

These different procedures work to restrict amount of food intake at one time, to decrease absorption of digested food and changes the level of gut hormones so patient gets early feeling of satiety (decrease hunger) and increase the metabolism of glucose and fats. Increasingly, hormonal changes are being recognized as an important mechanism of post surgical weight loss.


  • Bariatric surgery done laparoscopically (by making hole) like other Gastro-intestinal Surgery now a days and this technique popularized  Bariatric Surgery .
  • Small holes made over the abdominal wall and insert instruments into the abdominal cavity, with guidance of a surgical telescope with a video camera  and real time video seen over monitor to perform surgery under General Anaesthesia.
