Dear Patient: You need to tell me what symptoms your mother had initially and having it now?
Any fever or flank pain, vomiting etc?
Any comorbidities like DM?
Any similar episodes in past?
Was any imaging study, urine culture was done?
Any other medical history in past ?

UTI is very common in elderly more so in elderly females.. usually it's limited to bladder infection ..
Need to get one urine routine and culture and ultrasound KUB done ..
Initially response to niftas and then recurrence suggests that likely some microbe is resistant to this drug ..may need to upgrade antibiotics..
Can start Levoflox 500 mg once daily after giving urine culture..
