First of all I want to tell you that you are so much stressed with simple problems..You are a healthy young man and be busy with work and enjoy the essence of life in a healthy way.

Now about your queries:
1.You can take these combination, no serious interaction has been reported yet.
2. Anal fissure usually presents after chronic constipation and with drops of fresh blood in stool and severe pain..I hope your symptom is not so typical...You better consult a general surgeon for examination of that region.
3. For heavy breath on going upstairs. Is it associated with any chest pain? But your ECG is normal..You can consult a cardiologist. Further tests like TMT, ECHO can find out where the problem is.
4. As now your LFT is normal your liver is not in serious risk. You can take all your ongoing medications.. If you are going to take any new drugs please consult your physician.
5. Your medicines are perfect. Continue those. I shall advise you to consult an psychiatrist and do a counselling session under him/her. I think it will be of great help for you.
