Missed Period – Pregnancy Test Negative

·         It is a surprise to any girl or a woman in the reproductive age to have missed her period. .

·         In a woman of a reproductive age group, the most common cause for a missed period is pregnancy. But real worry starts when the pregnancy test is negative.

·         Missed period with negative pregnancy should be evaluated comprehensively under a gynecologist’s guidance.

·         To make the approach easier, the possible causes of missed period can be listed according to women’s age.

Pubertal Girl

·         Missed period is the most common complaint in adolescent girls. They may present with missed periods and irregular cycles every 3 to 6 months.

·         Another common cause is hormonal imbalance as HPO axis (Hypothalamic Pituitary Ovarian) is immature in this age, regular rhythmic hormonal secretion is missed & cycles are anovulatory ( No Ovulation )  & irregular.

·         Severe degree of hormonal imbalance may lead to PCOD ( Polycystic Ovarian disease )

·         Large Ovarian Cysts / dermoid cysts also cause hormonal imbalance leading to missed period.

·         Hypothyroidism (Low Levels of Thyroid Hormones) present with missed period.

·         Hyperprolactinemia ( High Levels of Prolactin )

·         Chromosomal defects – Turners Syndrome (45×10) present with total absence of periods.

·         High-Grade Viral Fevers.

·         Severe Degree of malnutrition.

Reproductive Age

·         Missed Period with Negative Pregnancy is not so common in this age group. But conditions to be considered as:

1.     PCOD – Altered lifestyle, eating & sleeping habits, physical & mental stress. All these reasons cause altered hormonal rhythms in missed periods with irregular cycles.

2.     Other conditions could be low thyroid hormones, high prolactin, high-grade fevers, severe malnutrition, ovarian cysts, chronic diseases like leukemia, women with progesterone coated IUCD (Mirena).

Menopausal Age:

·         Missed period is again a common complaint in perimenopausal women.

·         The waning effect of ovarian hormones is the basic change.

·         Other diseases like leukemia, high fevers, and malnutrition may exist.

·         Irrespective of the age of presentation, proper evaluation should be done including USG abdomen, hormonal assessment, thyroid profile, prolactin, FSH & LH, testosterone, estrogen, progesterone, basic blood investigations etc.

·         History & investigations lay a road towards diagnosis, which should be addressed.


·         Counseling plays an important role in treating these patients.

·         Pubertal girls should be explained & assured about the hormonal rhythms & advised to follow a healthy lifestyle.

·         Reproductive age group should be advised lifestyle modification following regular exercise, dietary restrictions and avoiding physical & mental stress.

·         Menopausal women should be counseled about the transitional changes accruing towards menopause.

·         Second line of treatment depends on basic pathology. Hormonal pills can be used to correct  the hormonal imbalance. Correction of nutrition, supplementations of the thyroid hormone and in a rare scenario, large ovarian cysts may require treatments.

A Comprehensive approach will reassure & correct the problem in all age groups.
