With a history of hypertension and T2DM, if he is feeling restless, you need to get his BP checked immediately, and FBS/PPBS evaluated.

Left shoulder pains could be due to periarthritis shoulder, which is a common manifestation in T2DM. You need to get an X-RAY of the left shoulder to check for evidences of arthritis and if required start medicines accordingly.

Kindly get his BP and FBS/PPBS checked as soon as possible.
