Rough patch may be anything ranging from submucous fibrosis to cancer.As you have said you take guthka, it might just be a submucous fibrosis , a benign (not harmful) condition. You need to mention about some things here, like
1. Since how long have you been taking guthka/or chewable tobacco?
2. Is there any bleeding from the rough patch?
3. Has it grown significantly in size within a few months?
4. Is it painful?
I suggest you meet an ENT specialist and do accordingly.
